In order for your child to participate in an Athletics class, the following items must be completed by Friday, August 16. Physical & Medical History must be completed by a medical doctor and turned in to Coach Lee or Coach Dansby 2. Fill out the 4 Electronic Forms on the website. **This is the MISD Athletic Department online paperwork. It is different from the paperwork you fill out to register your child for school.** 3.
